Neighborhood Health Association Partnering with A Renewed
Mind to Provide Innovative Care in a Patient-Centered
Medical Home

In efforts to support the
expansion of substance abuse services, with a focus on
Medication-assisted Treatment (MAT) for those with opioid
dependencies, the Health Resources and Services
Administration (HRSA) awarded $352,000 to Neighborhood
Health Association (NHA) to provide innovative and
integrated care in the Greater Toledo area.
With the award, NHA will
partner with one of the leading substance abuse service
providers in the region, A Renewed Mind (ARM), to
significantly expand access to high quality substance abuse
treatment services under an integrated care model.
Under this awarded
project, NHA will continue to provide the full range of
required primary care and preventive medical and dental
services for the full life cycle, and begin to offer
Medication Assisted Treatment (MAT) for opioid dependency. A
Renewed Mind, NHA’s partner in this initiative, will deliver
care coordination, counseling, in-person and app-driven
recovery support and other services.
NHA’s 13 health centers
located throughout the area will serve as an “on ramp” or
entry point for individuals with substance use disorders. In
addition, all service delivery sites operated by project
partner, A Renewed Mind, will also serve as entry points for
individuals with substance use disorders that do not have a
primary care home or would otherwise benefit from integrated
primary care/behavioral health services.
To date, no other providers in this area that work with this
population, offer fully integrated primary care/addiction
medicine services that fully incorporate MAT. The model that
will be employed by NHA and ARM will be the first such
integrated delivery model in the region.
The Health Resources and Services Administration is part of
the U.S. Department of Health and Human Services. HRSA is
the primary Federal agency responsible for improving access
to health care services for people who are uninsured,
isolated, or medically vulnerable.
